Post-Activity Survey

At the completion of a scholarship funding period, we would ask that you fill out a survey to let us know how the scholarship is going so far. There are questions for both the parent and the child, and we ask that you answer the survey questions honestly. Let us know how the scholarship process and activity experience are going so far. This helps us determine which leagues to refer kids to.
